Application for Enrolment Forms

To apply for enrolment at Rockingham Montessori School, please complete the age-appropriate application form below, attach the listed documents, and pay the non-refundable Application Fee of $110.

Payment may be made into the School’s bank account (BSB 036 074 Account 673 288). Please write your surname and initials as the reference and email reception@rms.wa.edu.au with the payment details. Alternate methods of payment are cash or EFTPOS in Reception, Visa or Mastercard over the phone or cheque.

Infant Toddler Program (up to age 3)

1. Please complete Infant Toddler Program Enrolment Form
2. There are no required documents to attach
3. There is no Application Fee

Primary Program (age 3 to 12)

1. Please complete Primary Program Application for Enrolment Form 
2. Please attach copies of the following documents:
   a. Birth certificate
   b. Australian Immunisation Statement
   c. Visa (if applicable)
   d. Past school report (if applicable)
3. Please pay Application Fee (non-refundable) of $110

Adolescent Program (age 12 to 18)

1. Please complete Adolescent Program Application for Enrolment Form
2. Please attach copies of the following documents:
   a. Birth certificate
   b. Australian Immunisation Statement
   c. Visa (if applicable)
   d. Past school report
   e. Transcript (former school’s teacher/principal recommendation)
   f. Parent recommendations
   g. Student questions and essay
3. Please pay Application Fee (non-refundable) of $110
